Formerly known as Supportive Living 4 or SL4. According to the plan of care, a higher level of personal support is provided, and healthcare services are offered onsite for scheduled and unscheduled care needs. SL4 is suitable for individuals with more complex medical needs that are predictable and safely managed with onsite, professional nursing (LPN level) and the direction of the case manager.
This level includes three meals daily, housekeeping services, access to our activity programming and 24-hour emergency response. It also includes assistance with the activities of daily living and additional unscheduled care.
